In the last two years, over 2,000 people have shared their experiences of leaving care with Healthwatch.

Summary

When discharge goes wrong, it comes at significant cost, both to individuals and to the health and social care system.

Since the publication of Safely home, the number of beds lost as a result of has risen from 4,754 beds in July 2015 to 5,861 beds in July 2017, a rise of 23.3%

In order to further understand people’s experiences of leaving care in light of these increases,we’ve brought together a summary of the experiences over 2,000 people have shared with 46 local Healthwatch during the last two years.
